Hours Of Work: 20 - 32 Hours Per Week; 5 Days Each Week; 8 Hours Each Day; 8:15 Am - 5:00 Pm; Monday - Sunday; Could Require Occasional Evenings or Weekends

CHW Equitable COVID Outreach Job Description
Community Outreach: Plays an integral role in educating the public, especially in communities where there is large vaccine hesitancy and information gaps, about Covid-19 and the Covid-19 vaccine.
Responsible for outreach both in the community and with local partners and businesses including schools, churches, and stores. Will perform a variety of outreach activities including door-to-door flier distribution, meetings with local residents and partners, health fairs, and community events. Knows and builds relationships with local community members and leaders to determine health needs of the community and to build trust.
  • Builds and maintains relationships with local residents through door-to-door outreach, involvement in community organizations and outreach log documentation
  • Provide education on COVID-19 prevention, testing, vaccination and support services
  • Attend testing and vaccination events to provide outreach and support
  • Participates in the coordination of regular health promotion activities such as local community events, organizational events and health fairs
  • Builds and maintains relationships with local school representatives, organizations, businesses
  • Refer community members to services, resources and information based on their stated needs
Health Resources Navigation:
  • Works with community partners to identify and register individuals who are interested in receiving the vaccine yet have difficulty with technology or internet access. Also, helps to connect individuals with transportation to and from vaccine events.
Community Advocacy:
  • Assists in sharing community feedback information to local health district vaccine leadership team to inform vaccine communication and event planning.
  • Serves as a community connection point for convening community residents around public health programs research interventions, and community assessments.
  • Advocates for community change through participation in City and Statewide initiatives.
  • Attends and encourages community residents to participate in local meetings and community surveys to ensure community voice in local decision making.
  • Gather information from community members about vaccine hesitancy or access concerns and work with other Crater Health District staff to develop and implement strategies and solutions
  • Performs all duties and responsibilities with a trauma informed approach.
Team and Professional Development:
  • Coordinates activities with supervisor to assure optimum use of time and effort.
  • Attends staff meetings and community meetings.
  • Participates in in-services, workshops, and classes to maintain knowledge, skills and abilities (ksa's).
  • Attends program-required training/educational sessions.
  • Identifies and requests attendance at appropriate educational programs to enhance job performance. Maintains records of trainings and submits documentation to supervisor.
  • Participates in team processing sessions to discuss successes/challenges in program implementation, relevant current events, community feedback, resources, etc.
  • Participates in all team meetings and staff meetings, and retreats
  • Assists with grant application preparation and reporting as requested
  • Participates in team photos, press briefings and media appearances when requested
  • Works with LHD programs/staff and/or community partners to implement programs/activities related to identified categories
Work Assignment as Described in Incident Response Plans:
  • Perform community health services for cases, contacts, and community groups both in-person and telephonically.
  • Communicate and support self-isolation and/or quarantine protocols to individuals and families.
  • Assess the ability of cases/contacts to access food, groceries, medication, and ability to isolate/quarantine via phone interview.
  • Refer cases/contacts to appropriate resources if unable to isolate/quarantine in current living conditions.
  • Select CHW staff will support the immediate needs of Cases/Contacts by aiding with food/medication access, as well as delivery of Wellness Kits.

Minimum Qualifications
  • Any combination of education, experience and training equivalent to graduation from high school or GED issued by state department of education
  • Three years of work-related health and human services or relevant community volunteer experience, such as with parent groups, faith-based organizations, civic groups, etc.
  • A related two-year (Associate) degree may be substituted for the required experience with culturally diverse individuals during a time of crisis and distress
  • Ability to show empathy to distressed individuals
  • Ability to handle confidential information with discretion and professionalism
Preferred Qualifications:
  • Certified community health worker
  • Experience in community education/outreach; knowledge of public health topics such as health equity, health disparities and social determinants of health
  • Ability to work with people from diverse backgrounds and experiences
  • Ability to work independently including organizing and prioritizing activities
  • Ability to communicate clearly and concisely both orally and in writing
  • Ability to work some nights and weekends
