  • Knowledge of behavioral health principles, techniques, and practices, and their application to complex treatment and services provision;

  • Ability to interpret and apply guidelines and procedures and maintain quality control standards;
  • Skill in establishing rapport with clients in discussing sensitive issues in a supportive and nonjudgmental and culturally agile way that also incorporates principles of Trauma-Informed Care;
  • Ability to establish and maintain effective working relationships with community members and provider agency staff is necessary; and
  • All job duties must be performed in a manner that demonstrates understanding of the basics of HIV Care: the HIV Care Services basic of continuous improvement, respectful communications, customer service and support of the unit's mission and values.
EDUCATION:
Master's degree in social
work (MSW) - Preferred.
Graduate of an accredited college or nursing school as a registered nurse - Preferred.
Bachelor's degree in Social Work plus four years' employment in a medical or health care setting, social service agency, or community organization focusing on health.
This position requires excellent customer service skills, strong computer skills, collaboration and critical thinking. Experience working with the health care delivery system.Mandatory skills/certification(s) Requirement:
Bilingual in Spanish or Amharic needed. Desired skills:
Virginia's Medication Assistance Program (VA MAP) provides access to medications for the treatment of HIV and related illnesses for low-income clients by providing medications or assistance with insurance premiums and medication co-payments as a payer of last resort.
The Eligibility Care Coordinator Team Leader will assist the Assistant Director for Medication Access in developing infrastructure and processes to aid the Eligibility Care Coordinators in successfully meeting quality improvement goals. This position is responsible for affecting ongoing quality and efficiency by applying a proactive approach to all eligibility care coordinators' daily operational duties.
The Eligibility Care Coordinator Team Leader will supervise Eligibility Care Coordinators: five coordinators assigned to a specific Virginia health region (Central, Southwest, Eastern, Northern, and Northwest) and one floating coordinator who will fill gaps in larger areas.
The Eligibility Care Coordinator Team Leader will be responsible for reaching the ADAP eligibility projected goals, and duties will include:
Establishing a relationship with client and assessing client's health needs
  • Collaborate with a multi-disciplinary team including ADAP team and case managers to identify problems or needs that require special intervention, education or follow-up;
  • Build a trusting, effective relationship between the Eligibility Care Coordinators and case managers;
  • Review necessary ADAP eligibility and recertification documentation received from staff;
  • Identify key problems, strengths or assets that help execute tasks, and resources in geographic areas.
Monitoring
  • Monitor client compliance regarding certification and recertification;
  • Prepare and maintain eligibility care coordination reports, and prepare periodic reports for senior management decision making purposes, as required;
  • Monitor staff performances and client's compliance to the established care plan interventions; and
  • Monitor and revise care plan, as the client's needs change.
Reassessing the RW eligibility status
  • Perform record review and share findings with staff;
  • Perform related work and additional duties as requested by supervisor; and
  • Under supervision of Assistant Director for Medication Access, communicate results of program evaluations to HCS team and stakeholders in appropriate manner.
Improving Quality of Care
  • Serve as subject matter expert in quality and value based ADAP programs;
  • Access specific ADAP data portals to obtain data for preparation of reports and other documents to evaluate the progress of quality programs;
  • Under the supervision of the Assistant Director for Medication Access, assist in the development of tools, education and workflow processes to assist the unit in meeting the program's projected goals;
  • Collaborate with interdisciplinary teams and leaders to achieve HCS unit's coordination of performance goals;
  • Participate in the HCS QM Team;
  • Utilize benchmarking and other sources to identify best practice;
  • Participate in setting performance targets to improve quality; and
  • Promote and facilitate effective communication through the continuum of care
