-Consults with patients/reviews medical history and referrals on individuals' symptoms, diagnoses; develops and implements a treatment plan.
-Evaluates and initiates skilled Speech/Swallow Therapy treatments in accordance with provider orders.
-Serves as a member of a treatment team; attend care plan meetings, communicates physical therapy treatment plans to family members as needed.
-Assist individuals with the use of equipment needed for eating, drinking and communication strategies.
-Recommends diet consistencies and dining techniques.
-Teach and coach speech and/or swallow exercises as appropriate.
-Observe and review individuals' progress and adapt treatment accordingly.
-Supervise and train new Speech Language Pathologists, review and cosign paperwork, as needed.
-Maintains communication with providers.
-Educated staff in choking prevention, dysphagia, aspiration and communication.
-Performs and completes paperwork and billing in a timely manner.
-Participates with interdisciplinary communication and collaboration.
-Updates Residents' care plan when changes are noted.
-Participates with discharge planning.
-Participates with identifying and ordering equipment needed.
-Professional interaction with residents, coworkers, visitors, and families.
