4-We also require a current (within the last year) TB skin test results or if they have had a past positive TB test we will need a clear chest x-ray report and/or a physician check list or certification (signed by the physician) that they are free of Symptoms of TB

LPN Duties:

Pull and give meds and sign MARx 3 (7a, !Ia, and 4p) .
Treatments before breakfast.
Check chart before clinic.
Morning briefing, when RN is not available.
Monthly vital signs, chart and fax to dietmy.
Monthly weights
Nursing reviews 5 to 13 monthly .
T.B. skin test (chart them) and read them in two days.
Vision acuity (check chart for ammal eye exam.)
Chart individuals in and out on clinic trips .
Note orders when return from clinic trip .
Fax orders to pharmacy .
Chart checks .
A&Is for individuals and staff- assessments and follow-up treatment.
Watch inservices .
Answer phones and respond (i.e. seizures) .
Stock and clean medication room (mop, take out trash, and wipe down counters).
Prepare passes for take-home meds .
Complete body audits and weight for home visits
Assist doctor.
Assist RN when needed .
Note orders.
• Check medication administration records (MAR).
• Check pre-MARs.
Provide wound care and treatment.
• Review clinic reports daily.
• Re-fill/order meds pm.
• Administer insulin.
• Accuchecks.
• Neurochecks and Glasgow.
• ER visits (paperwork, phone calls, VS).
• Medicalmemos.
• Consults.
• Diet orders.
Family contacts
• Administer PRN meds and chart, follow-up and chart.

CHARACTERISTICS OF WORK:
This is technical work involving participation in and implementation of nursing care in all settings. The work is performed under the supervision of a licensed physician, dentist, or Registered Nurse. Supervision is frequently exercised over attendants or other workers in lower classifications.

EXAMPLES OF WORK:
Examples of work performed in this classification include, but are not limited to, the following:
Takes and records vital signs, reporting significant changes to the appropriate person.
Prepares equipment and aids physician during treatment and examination of patients.
Performs tracheotomy care, catheterization, catheter care, and gastrostomy care.
Assists with rehabilitation of patients.
Prepares food trays and assists patient with diet as needed.
Provides for the emotional and physical comfort and safety of patients.
Observes, records, and reports to the appropriate persons the general and specific physical and mental . conditions of patients.
Renders general nursing care to patients by administering prescribed medications and treatments in accordance with approved nursing techniques.
Provides direct patient care under supervision of the Registered Nurse, physician, or dentist.
Performs related or similar duties as required or assigned.

MINIMUM QUALIFICATIONS:
These minimum qualifications have been agreed upon by Subject Malter Experts (SMEs) in this job class and are based upon a job analysis and the essential functions. EXPERIENCE/EDUCATIONAL REQUIREMENTS:
Education:
Graduation from a standard four-year high school or equivalent (GED or High School Equivalency Diploma);
1 AND
Education:
Completion of an accredited one-year (thirty (30) semester hours) practical nursing program;
AND
Experience:
Two (2) years of experience working as a Licensed Practical Nurse and licensure as required below.
Certification/Licensure Requirements:
' Must possess a valid Mississippi License of Practical Nursing.
Documentation Required:
Applicant must attach a valid copy of his/her Mississippi License of Practical Nursing.

ESSENTIAL FUNCTIONS:

Taste/Smell: Possesses the ability to use the sense of smell to recognize and distinguish odors.

Motor Coordination: While performing the duties of this job, the incumbent is regularly required to walk;
use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or bend. The incumbent is frequently required to stand. The incumbent is occasionally required to sit.
